Guest Questionnaire

The following client questionnaire is information that we request from all clients that visit our SPA. It can be filled out in advance and brought with you to your scheduled appointment so you can enjoy your time with us to the fullest extent. If you wish to fill it out at the SPA, we need you to arrive 20 minutes prior to your appointment to do so. If you are coming for Skin Care, please fill out the skin care form on page 3 of the questionnaire. You MUST fill out the Client Consent Form on page 4. This is required by the state and must be signed by our therapist that sees you.
